If your needs align with the services offered in this practice, you will receive an invitation to the patient portal to complete intake paperwork and schedule an initial appointment. If we are not able to accommodate your needs at this time, we will contact you by email and, when appropriate, share recommendations for other resources.
Please note: we do not accept insurance at this time. Payment is due at the time of service, via credit/debit card or check. Patients are welcome to submit claims to their insurance carrier for potential out-of-network reimbursement, and we are happy to provide appropriate documentation to support that process.
